Nursing Care - Required Documents
The Nursing Care residence status is for activities in which a person who holds a Certified Care Worker (Kaigo Fukushishi) qualification engages in nursing care or nursing care guidance based on a contract with a public or private organization in Japan.

Important Notes Before Applying

- If the application is submitted with incomplete documents, the examination may be significantly delayed or the application may be denied.
- All certificates issued in Japan must be submitted within 3 months of the date of issue.
- Documents written in a foreign language must be accompanied by a Japanese translation.
- Submitted materials will not be returned in principle. If you wish to have them returned, please inform the office at the time of application.
- You may be asked to submit additional materials during the examination process.
Certificate of Eligibility Application (New)
This application is for those who wish to enter Japan with a Nursing Care residence status for the first time.
1. One copy of the Certificate of Eligibility Application Form
2. One photograph
4 cm (height) x 3 cm (width), taken within 6 months before the application, front-facing, without hat, plain background. Write the applicant’s name on the back of the photo and attach it to the photo section of the application form.
3. One return envelope
Write the recipient’s address on a standard-sized envelope and affix stamps for simplified registered mail (404 yen).
4. Copy of Certified Care Worker Registration Certificate
5. Employment Conditions Notice
A document specifying the working conditions issued to the worker based on Article 15, Paragraph 1 of the Labor Standards Act and Article 5 of the Enforcement Regulations of the same Act.
6. Overview Materials of the Inviting Organization
Whichever of the following applies:
- (1) A brochure detailing the history, officers, organization, business activities, etc. of the workplace
- (2) Other documents prepared by the workplace equivalent to (1)
7. If working based on a dispatch contract (only if the applicant is a dispatched worker)
Materials specifying the details of the applicant’s activities at the dispatch destination (Employment Conditions Notice (employment contract), etc.).
8. Notification Regarding Transfer of Skills (only if applicable)
This must be submitted only if the applicant has previously stayed in Japan under the “Technical Intern Training” residence status.
Change of Residence Status Application
This application is for those who are already residing in Japan under a different residence status and wish to change to the Nursing Care residence status.
1. One copy of the Change of Residence Status Application Form
2. One photograph
4 cm (height) x 3 cm (width), taken within 6 months, front-facing, without hat, plain background. Not required for those under 16 years of age.
3. Passport and Residence Card
Present them (if unable to present a passport, a statement of reasons explaining why).
4. Copy of Certified Care Worker Registration Certificate
5. Employment Conditions Notice
A document specifying the working conditions issued to the worker based on Article 15, Paragraph 1 of the Labor Standards Act and Article 5 of the Enforcement Regulations of the same Act.
6. Overview Materials of the Inviting Organization

Extension of Period of Stay Application
This application is for those who are already residing in Japan under the Nursing Care residence status and wish to extend their period of stay.
1. One copy of the Extension of Period of Stay Application Form
2. One photograph
4 cm (height) x 3 cm (width), taken within 6 months, front-facing, without hat, plain background. Not required for those under 16 years of age or for extensions of 3 months or less.
3. Passport and Residence Card
4. Residence Tax Taxation (or Non-Taxation) Certificate and Tax Payment Certificate
Must include the total annual income and tax payment status. Issued by the ward office, city hall, or town/village office of the place of residence as of January 1. If a single certificate includes both the total annual income and tax payment status (whether taxes have been paid), only one needs to be submitted.
Additional Documents for First Renewal After Changing Jobs
If you are applying for an extension of stay for the first time after changing jobs, the following documents must also be submitted in addition to the above.
